Successful businesses are successful because they focus on and improve the system. When a failure occurs, the focus is on the system to prevent it from happening again AND improve performance. Think of this from a safety perspective: instead of focusing on the failure as an act, focus on the failure as how the choice was made by the human within the system. Imagine what will happen if we use that data to prevent AND improve performance.
